Project Proposal Application Guidelines
Please read the application in its entirety before you begin providing
the requested information. Gather the information necessary to respond
to each item and be as concise and clear in your responses as possible.
The application will be scored using the criteria outlined below and
no additional information or attachments will be accepted. For ease
of distribution to the raters, it is required the application be submitted
electronically. Scoring will be determined using the attached evaluator’s
rubric (Appendix A). The maximum achievable application score is 100
points with the maximum “Item” score listed in parentheses.
Applications will not be accepted beyond the close of business on August
1, 2008.

Item I – Mission and Services (10 points)
Provide a brief overview of your organization’s mission
and the key services provided to the community.
Include:
•target population(s) served
•annual # of clients/customers served
•# of years your organization has served the Mohawk Valley
•scope of services provided
Item II – Previous Project Award History (10 points)
Although the previous award of an LMV project does not prevent you from
seeking another project, priority scoring will be given to an organization
that has either never received a project or has not had one in three
or more years. Scoring criteria for this item is defined in the attached
rubric.
Item III – Definition of Need/Problem/Issue (30 points)
We recognize that it is sometimes difficult to understand or identify
the exact need/problem/issue. However, you should outline as clearly
as possible the nature of the need/problem/issue based on your current
understanding of the situation.
Include:
•nature of the need/problem/issue (i.e., census building, financial,
marketing, outreach, services, technology, organizational, etc.)
•reason(s) why this need/problem/issue currently exists
•
definition of
how this need/problem/issue impacts organizational effectiveness
•identification as a local, state-wide or national organizational/company
need/problem/issue
•how long need/problem/issue has existed
•target population, if any, impacted by this need/problem/issue
•level of importance in addressing this need/problem/issue this year
Item IV – Outcomes (20 points)
Describe the desired outcome(s) e.g., qualitative or quantitative measurements,
you seek to achieve through the assistance of the LMV project team.
LMV makes no guarantee or implied promise as to the expectation or delivery
of specific outcomes.
Include:
•specific improvements to be gained in the areas of : census building,
financial, marketing, outreach, services, technology, organizational,
etc.
•anticipated # of individuals/clients/agencies who will directly benefit
•identifiable short-term versus long-term impact you seek to achieve
from outcome(s)
•level of impact on the Mohawk Valley region
factors that could hinder or enhance successful implementation (i.e.,
budgetary, Board/Administrative support, •degree of organizational autonomy
or flexibility, etc.)
Item V – Leadership Development (20 points)
A key requirement of LMV is to insure that team members acquire valuable
leadership development experiences to include team building skills,
problem solving skills, assessment skills, communication skills, conflict
resolution skills, and presentational skills. Understand that project
team members bring strong skills and knowledge to the table. As such,
they are to be treated as “consultative experts” and should
not be seen or utilized as additional personnel to shore up your organization’s
staffing issues. Describe how this project will provide opportunities
for such development to take place.
Include:
•specific challenges (perceived or real) team members may face
•specific information (written and/or oral) to be provided to the team
•amount and type of research, examination and assessment expected/anticipated
complexity of activities
Item VI – Timeframe (10 points)
Another important requirement will be the timeframe in which your need/problem/issue
can be addressed. It is essential that the project team has sufficient
time to conduct an assessment, to collect information, to decide on
a course of action, to seek the organization’s approval and,
time permitting, move forward with an action proposal and/or implement
of recommendations.
Include:
•projected timeline that meets the September – May requirements
•availability of project liaison to the project team
•plan to maintain regular contact with LMV project team members
•restrictions or limitations (i.e., special events, workload issues/work
schedules or other foreseeable circumstances) that the project team
will need to take into consideration
Additional LMV Requirements (0 points)
Although no points are awarded for this section, in order for your project
application to receive consideration all three criteria must be met – acknowledged
by placing an “X” in each of the three boxes. It should
also be noted that a fee (anticipated at $25/person) will be charged
to cover the cost of the Graduation luncheon, which serves as the forum
for the project presentations.
