   E A C H   Y E A R . . . . LEADERSHIP MOHAWK VALLEY will honor outstanding community leaders at the Annual Follow the Leader awards dinner. Nominees are usually graduates of LMV and have demonstrated excellence and leadership in the workplace and/or the community. Individuals who have not graduated from LMV may be considered based on their participation with LMV and overall contributions to the community. LMV’s selection committee has the responsibility of evaluating and selecting the award recipients. Selections are based solely on the information provided from reviewed criteria received through the nomination forms.

Criteria:
A. How does this leader personify LMV’s mission of “developing community growth and excellence”?
B. List the leader’s major responsibilities and scope of position in the workplace/community
C. What personal qualities/traits distinguish this leader, and what has been their impact to the community/workplace?
D. What challenges has this leader overcome while trying to succeed?
